Welcome to Health Shift. These Terms & Conditions (“Terms”) govern your use of services provided by Cordero Chiropractic APC DBA Health Shift (“Health Shift,” “we,” “our,” or “us”).
By scheduling an appointment, using our website, or receiving services, you agree to these Terms.
Services Provided
Health Shift provides chiropractic care, rehabilitation services, fitness-based recovery services, and wellness support. Services are provided in person at our clinic.
Health Shift does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or seek immediate medical care.
Appointments & Scheduling
Appointments are required for services.
Cancellation Policy
We require 24 hours’ notice to cancel or reschedule an appointment.
Appointments canceled with less than 24 hours’ notice may be billed at the full session rate.
Late Arrival Policy
If you arrive late, your session will be completed within the remaining scheduled time. The session fee remains the same.
Payment Policy
Payment is due before or at the time of service unless otherwise arranged. Packages and prepaid services may be offered.
Refunds are handled on a case-by-case basis at our discretion.
Injury Risk & Liability Acknowledgment
Participation in chiropractic care, rehabilitation exercises, and fitness-related services involves inherent physical risk.
By receiving services, you acknowledge that:
You voluntarily participate in activities and services
You understand physical activity carries risk of injury
You assume responsibility for your participation
You will communicate any pain, discomfort, or concerns to your provider
Health Shift is not responsible for injuries resulting from failure to follow professional guidance or failure to disclose relevant health information.
SMS terms of service
By providing your phone number and opting in, you agree to receive SMS messages from Health Shift related to appointments, services, and Health Shift programs.
Message frequency varies. Message and data rates may apply.
Reply STOP to unsubscribe. Reply HELP for assistance.
Wireless carriers are not liable for delayed or undelivered messages.
You must be 18 years or older to opt in to SMS messaging.
